Final Practicum Evaluation (Confidential)

Please fill out the confidential evaluation form, below, one week before the student's practicum is finished. When finished, click the submit button at the bottom of the form. You will receive an email copy of this submission.

Please use this final evaluation as an opportunity to discuss the practicum experience in a summary way with your practicum student(s). This is a good opportunity for you both to achieve positive closure on this experience.

While university regulations permit only the SIS Practicum Coordinator to officially assign a grade for a student taking the Practicum (IS 599), your appraisal and recommendation of a grade as the practicum supervisor is given the greatest possible weight in determining that grade.
